Registering for Library Services
In order to use the library services you must:
- Be an eligible staff member of the Department of Health in Western Australia
- Be entitled to use the services of this particular library. If you are not sure check our Frequently asked questions - Which library to use?
- Become a registered library borrower so that we have your contact details
- Provide us with a Personal Identification Code that you will use as your electronic signature on the online forms and
- Provide us with a declaration in relation to Copyright
The registration forms below each include the required information and declarations. There are two forms, please select your staff group.
- Royal St Divisions staff, including staff at Health Corporate Network, Health Information Network, Grace Vaughan House
- WACHS staff
- Osborne Park Hospital and Swan-Kalamunda District Hospital staff
- Ambulatory care staff in the North Metropolitan Area Health Service
- Dental Health Services staff
- Approved non-health staff providing services to the Department’s clients. This includes all Visiting Medical Officers. Authorisation from a Departmental officer is required. Please ensure authorising officer’s name is clear and legible.
- A student enrolled in a departmental course. However, medical students attached to a hospital should apply for services at that hospital’s library. Authorisation from the course co-ordinator is required.
